About the Program

PROGRAM OVERVIEW

The Schools for Healthy Lifestyles (SHL) program was developed in 1997 to promote and maintain healthy lifestyles among children, families and school faculty. Participation in the program benefits not only students but your entire school community. The founding partners of SHL are Oklahoma County Medical Society, Oklahoma City Public Schools, Oklahoma City-County Health Department and the State Department of Health. The program is also endorsed by the Oklahoma State Department of Education.

Schools selected to enter the School for Healthy Lifestyles program focus on five key areas which are related to reducing the leading causes of poor health among Oklahoma residents. Schools follow a tiered program approach which encompasses SHL's five focus areas:

 
Freshman (1st year) - $1,000 Grant – Physical Activity and Nutrition focus areas will be implemented.
 
Sophomores (2nd year) - $750 Grant – Injury Prevention and Tobacco Use Prevention will be implemented.
 
Juniors (3rd year) - $500 Grant – Oral Health Education will be implemented.
 
Seniors/Certified (4th-8th year) – Up to $500 Grant – Schools will become eligible for SHL sponsored programs such as Adopt-A-Doc, Adopt-A-Dentist, Wacky Water Wahoo, bicycle rodeos, etc.
 
Alumni (9th+ year) – Up to $500 Grant – Schools will follow a modified requirement program but will still be able to access SHL resources and participate in all activities.
 

Benefit to Schools

SHL schools receive the opportunity to enhance the health knowledge of students, families, and school faculty plus provide educational intervention in the attitudes and behavioral skills required for the adoption of healthy lifestyles. Schools in the program also receive:

 
Up to $1,000 grant award   Health and physical education curricula
 
Public recognition, certificate and school flag   Assessment materials and Fitnessgram kit
 
Intense training at the Summer Health Institute   Ongoing technical assistance and support
 
Link to community resources and presenters   Lifeline newsletters for students, faculty and staff
 
Adopt-A-Dentist partnership   Adopt-A-Doc partnership

 

How to Apply

Your school must be a public elementary school, grades K through 5/6. (Due to public funding, private schools cannot be approved for participation.)

Prior to formally applying for the Schools for Healthy Lifestyles program, each new school must fill out a Letter of Interest. This can be done at any time by mail, fax, or online.

Letter of Interest:

Elementary schools, not currently participating in the SHL program, that wish to participate in the grant application process are required to complete this Letter of Interest by the deadline of January 20th.

We have read the above program information and are aware of the purpose and mission of SHL and how participation will benefit our school.
 
We understand that, if approved, we must designate an individual to serve as our SHL coordinator (PE/classroom teacher, school nurse, or counselor). We also understand that if our school has more than 700 students, two coordinators must be designated. A 3-member team from our school must attend the two-day Summer Health Institute teacher training to take place in late July or early August (dates TBA).
 
We understand that, if approved, our school must agree to implement the recommended program curricula and evaluation tools/equipment provided by SHL. (Valued at approximately $5,000)
 
We understand that, if approved, our school must agree to administer a pre/post Fitnessgram physical fitness test to all 3rd, 4th and 5th grades.
 
We understand that, if approved, our school must agree to administer a pre/post written survey to all 3rd, 4th and 5th.
 
We understand that, if approved, our school must use grant funds solely for purposes of promoting SHL's five focus areas: Physical Fitness; Nutrition; Injury Prevention; Tobacco Use Prevention and Oral Health.
 
We understand that, once approved, if our school should leave the SHL program (for any reason) before we are certified a school, all equipment and unspent grant funds must be returned to SHL.
